The New York State Medicaid Section 1115 Demonstration Waiver ("1115 Waiver") has improved access, quality, and cost effectiveness of health services for New Yorkers. The 1115 Waiver has allowed the State to implement its Medicaid Managed Care (MMC) program, which provides comprehensive and coordinated health care to Medicaid recipients. Our Social Support Navigation Program helps qualified individuals navigate this waiver and get access to care.

Eligible Populations
Members must be enrolled in Medicaid Managed Care (MMC) and fall into one or more of the following populations:
Medicaid High Utilizer;
Individuals enrolled in a NYS-designated Health Home;
Individuals with Substance Use Disorder (SUD);
Individuals with Serious Mental Illness (SMI);
Individuals with Intellectual and Developmental Disability (I/DD);
Pregnant and postpartum persons;
Individuals who are up to 90 days post-release from incarceration with a chronic condition, including SUD and chronic Hepatitis C;
High-risk children under the age of 18 -
Scope of Services We Can Provide
Enhanced care management: referral management, coordination of services, education, and follow up
Housing: medically necessary home accessibility/safety modifications, asthma remediation, medical respite, rent/temporary housing, utility assistance, pre-tenancy services, and more
Nutrition and nutrition counseling and education, food prescriptions, pantry stocking, medically tailored meals, cooking supplies, and more
Transportation: public or private transportation for use of services or referrals
